DiMi Nursing Employment Information

All applicants MUST apply in person.  We do NOT accept online applications.

We accept applications at our Boynton Beach office on Tuesday, Wednesday and Thursday between 10:00am and 3:00pm. Professional attire preferred.    

Please bring the following with you to fill out application

  • Florida Driver’s License
  • Social Security Card
  • Documents Authorizing Applicant to work in U.S (1 of the following):
    • Work Permit
    • US Passport
    • Certificate of Naturalization
    • Birth Certificate
    • Voters Registration Card
    • Permanent Resident Card
  • Physical (Dated within last 6 months and NOT performed by a Chiropractor) Must state applicant is “free of communicable diseases” and be signed by a M.D., AARNP, P.A. or D.O.
  • Chest X-Ray / TB Results or PPD
  • Level 2 Background Screening
  • Copies of Professional Certificates or Licenses
  • Initial HIV- 2hrs
  • Domestic Violence-2hrs
  • Assisting with Self Medication- 2hr certificate
  • Alzheimer’s- 2hrs
  • Infection Control
  • Proof of Automobile Insurance
  • Proof of Automobile Registration
  • Professional Liability Insurance ( Can be done during application process)
  • Two professional / work references with fax / phone numbers
